Hello and welcome to the Roosevelt Middle School Health Office page! My name is Heather Smith and I am the school nurse. Need Shots?

Attention parents! As you already know, keeping your child current with their immunizations is not only important, but also a state requirement for school enrollment. Letters have been sent home to families whose students still need shots. The UNM School Based Health Center at Van Buren Middle School is an ideal place to take your child to get caught up with their shots. To do so, please call the center at 505-256-2555 and schedule an appointment. You can leave a voice message if nobody answers. Parents can plan to walk in with their student to sign the consent form. Parents will then return to their vehicle and wait for their child to receive their vaccines. Vaccines are provided free of charge. If you have any questions or concerns, ask nurse Heather. Thank you! 

 

Covid testing Sites in NM

If you suspect that you or a loved one has COVID-19, you can check out this site to find the nearest testing location for COVID-19- https://cvprovider.nmhealth.org/directory.html. Type in your location to find one nearest you. You may also contact the New Mexico COVID-19 hotline at 1-855-600-3453 to ask any COVID-19 related questions. Leave a message and someone will return your phone call as soon as possible. Please report any positive COVID-19 tests for any student to the school, even during remote learning.

In need of clothes, jackets, or face masks for your student?

Contact me by phone at 505-281-3316 ext. 24107 or via email at heather.smith2@aps.edu if you would like a referral to the APS Community Clothing Bank. Services provided include new shoes, socks, underwear, jackets/hoodies, coats, and face masks. A referral must be made ahead of time, in order to receive these. With the cooling weather, now is an important time to make sure that children are warm enough.
